Social Services (LSW) Consultant

Position Description

The primary responsibility of this position is to fill in for The Director of Social Services or Social Worker FMLA and vacant positions and manage ongoing Social Services operations for the client.  You will be responsible to implement policies, procedures and forms that give structure to facility systems for regulatory and clinical standards compliance.  Responsible for providing services to respond to the emotional needs of the residents and their families.  You may also assist with resident admissions and the referral process.

Essential Job Functions and Tasks: 

  • Provide professional, quality-driven services including but not limited to: 

  • Mock surveys 

  • Research and education, including oral presentations 

  • Analysis of healthcare systems and processes 

  • Creative problem-solving and recommendations 

  • Written presentations to clients 

  • Function as an advisor for providers, agencies, and facilities 

  • Assist clients in assessing, planning, developing, and implementing systems, processes, and other needs as requested 

  • Consult and provide technical and/or interim management assistance to providers, as requested by client, including but not limited to: 

  • Interim social worker positions and/or interim Director of Social Services in skilled nursing facilities, home care agencies, assisted living facilities 

  • Identify training needs; plan and conduct training/in-services, as requested by client 

  • Attend applicable meetings at provider’s home office, as necessary 

  • Provide expert recommendations based on established and ongoing knowledge of federal and state statutes/regulations, research, network connections, and current interpretations of statutes/regulations by MDH surveyors 

  • Utilize AHI – Strategic Solutions principles of documentation and established templates/forms for all written community with clients and presentations  

  • Develop and maintain positive, working relationships with clients, network and industry representatives, and other AHI staff 

  • Provide oversight and manage the AHI social work consulting team as well as the AHI chemical dependency team  

  • Other duties as assigned 

Minimum qualifications: 

  • Master’s degree in Social Work is preferred, LGSW (Licensed Graduate Social Worker or higher); or a Bachelor’s degree in Social Work is required with current license in the State of Minnesota. 

  • Previous case management experience required, working in a skilled nursing facility for at least 1 year preferred. 

  • Valid driver’s license is required 

  • In-service training and education as required to deliver services
